If you are receiving services from Diamond Performance PT and Athletic Training, you will need to complete the document below. This documents will also be available to fill out at your first visit evaluation.

***WARNING*** If you are under the age of 18 and your parent or legal guardian is not coming with you to your appointment you will need your parents to print off and sign the document listed for you to bring to your evaluation in order to be seen.

If you do not have this document signed by a parent or legal guardian and you are under the age of 18 you will not be seen.
