If you are receiving services from Diamond Performance PT and Athletic Training, whether through a HumNow membership or directly, you will need to complete the documents below. These documents will also be available to fill out at the front desk.

***WARNING*** If you are under the age of 18 and your parent or legal guardian is not coming with you to your appointment you will need have your parents print off and sign the documents listed for you to bring to your evaluation.

If you do not have these documents signed by a parent or legal guardian and you are under the age of 18 you will not be seen.
